  Friday, Saturday, Sunday, Monday 7:00am - 5:00pm

To obtain your Key Card, please fill out the form below for faster service.
You can  then pick  up your card at our office at 8000 Columbus Road.
Card pick up times are now by appointment only, please call 330-875-6249.   
Please bring a drivers license or utility bill and picture ID
with you to prove your address.

Cash, Check, or Credit Card
(3% ($.75 cents) Convenience  fee for Credit Card purchases)
